NOTE for Lead: Any additional work outside of the contracted Scope of Work should be documented using this form. A separate AWV form should be filled out for each item of additional work. The Project Manager MUST be notified prior to the beginning or completion of any additional work. The process of utilizing this form should be established between the Lead and the Project Manager at the beginning of each project. At minimum, this form should be submitted within 24 hours.
